It can be very challenging to ensure the delivery of safe care within a hospital, primary or social care setting and mistakes can and do happen as a result of which a patient or service user suffers an injury or unfortunately dies.  This can be very traumatic for the loved ones of the patient but also for the healthcare professionals involved in that patient’s care.

Such serious adverse incidents can carry significant consequences, both personally and professionally, for the healthcare professionals involved and also from a repetitional perspective, for the organisation.
